Personal appeararance release form

All members if own focus group were asked to sign the form below , as we filmed our focus group and posted it on to utube. Therefore we needed the permission of them all to use them in our video. They were all sixteen and over , so didn't need parents permission.

PERSONAL APPEARANCE RELEASE

I hereby grant to Organizer(s) the right to photograph, film, videotape, or otherwise record me in connection with their Focus Group including the right to use my name, likeness, performance and appearance, and related biographical materials (collectively, "Image") in the Film, and to own the copyright therein, to use the Image, or any part thereof, throughout the world in perpetuity including, but not limited to, the right to exhibit, record, edit and otherwise alter at the sole discretion of the Organizer(s) the Image or any portion thereof, in any form of media whatsoever, without limitation or restriction, including for the purposes of advertising, publicity, promotion and trade in any medium whatsoever without restriction as to manner, frequency or duration of use without any future payments to me.

I agree that all right, title and interest including all copyrights, to any Image of me in the Film are owned by the Organizer(s).  No material needs to be submitted to me for any approval and the Organizer(s) shall have no liability to me for any distortion or illusionary effect involving my Image.

I represent and warrant that I have reached the age of majority and therefore can grant the rights hereunder or my parent or legal guardian will sign on my behalf and that the rights granted hereunder will not conflict with or violate any commitment, agreement, or understanding I have with other person or entity.

Nothing herein will constitute any obligation on the Organizer(s) to use any of the above rights.
